Fairview Pharmacy Services is hiring a full-time Clinical Pharmacist to provide clinical support and consultative services in a complex outpatient/infusion pharmacy operation at the M Health Clinic & Surgery Center, blocks from the University of Minnesota campus. This position partners with key stakeholders to drive medication safety, customer service, process improvement, clinical and formulary programs and initiatives and regulatory compliance. The Oncology Pharmacist will act as a primary resource in the development and education of students, residents, staff and interdisciplinary members of the care team. This is a great opportunity for a Pharmacist who has completed a PGY1 residency or a Pharmacist who has at least one year of experience in a complex pharmacy.
Provide clinical, formulary and pharmacotherapy expertise and act as a resource to students, staff and interdisciplinary members of the care team.
Act as resource for pharmaceutical care to patients This includes partnering with interdisciplinary care team members to ensure appropriateness of medication order and therapy, drug selection, formulary correctness and compliance any other formulary or departmental initiatives.
Provide clinical pharmacy services to all patients in assigned area including completing review for all patients daily, monitoring, adjusting patient therapies and documenting changes according to department policies.
Accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures and other extemporaneous orders as appropriate.
Effectively consult and educate patients, staff and students on drug
Execute the function of staff pharmacist

Lead Job Expectations:
Develop process improvement strategies for patients receiving oral chemotherapy across the system
Evaluate near miss patient database/Compass report and determine follow up plans
Implement new strategies that decrease overlap work between pharmacists and liaisons
Educate pharmacy learners in chemotherapy and treatment regimens
Design efficient workflow procedures to reduce patient touches but maximize patient wellbeing
Build a standardization system for patient interactions
Develop/monitor department workflow metrics to ensure that tasks are done in a timely and efficient manner
Cover for other offline project time for other Oral Chemotherapy staff when needed
Work closely with mangers from various departments completing work for oral chemotherapy patients
One year pharmacist experience in complex pharmacy operation or completion of an ASHP-accredited PGY1 residency program
Licensed or eligible for licensure as a pharmacist in Minnesota
Board Certification in Oncology Pharmacy
One-three years of pharmacy experience preferably in oncology or an ambulatory infusion setting or have completed a residency in an oncology setting.
