The Registered Nurse (RN) Referral Coordination Team is responsible for processing of pending specialty clinic referrals. The RN works together with Medicine and Health Administration Services to provide options for Veteran seeking care through the VA. The goal of the initiative is to keep the Veteran at the VA for services if eligible. Duties include but are not limited to: Determines Veterans enrollment status. Performs assessment of referral based on clinical triage. Contacts Veteran and provides options based on wait time and community care eligibility. If Veteran chooses community care RN will obtain preferences for same and forward to that department. Works closely with specialty providers at the medical center to overbook if there is availability. Educates medical center staff on Referral Coordination Initiative. Should be proficient using CPRS, CTM, HSRM, PPMS, RefDoc. Demonstrates consistent clinical competence in providing nursing care. Demonstrates evidence of critical thinking skills in identifying, analyzing, and resolving patient care concerns. Provides leadership in establishing goals, evaluating the plan of care, and in discharge planning. Initiates referrals to other disciplines and/or services as appropriate. Assesses the patient's learning needs. learning preference, and readiness to learn. Modifies teaching approach based on knowledge of human behavior, motivations. and reactions to situations. Evaluates and documents patient response to interventions and progress towards attainment of goals and expected outcomes. Revises interdisciplinary plan of care based upon on-going patient assessment and evaluation. Coordinate and support the needs of the Veteran requiring multidisciplinary teams in facilitating the disposition of a specialty consult. Remove non-value-added steps to optimize the use of clinical staff for the benefit of the Veteran. Maximize the interaction with the Veteran to identify needs and gather preferences to ensure that appropriate modality of care is offered. Identify possible barrier to care and with the Veteran, family and clinical care team to provide a team-based plan of care. Enhance the quality of clinical outcomes and Veteran satisfaction while offering the Veteran the opportunity to make an informed decision about their care.
