This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Learn more The Northern California VA Health Care System is currently recruiting a full time permanent GS-12 Social Worker (Program Coordinator) for the Community Nursing Home (CNH) program in McClellan, CA. Major duties include, but are not limited to: Manages the daily operation of the CNH program. Develops policies and procedures for program operation. Tracks program data. Organizes and prepares reports and statistics for the oversight committee, service leadership, the health system director, VISN 21, and VA Central Office. Receives, reviews, and takes action on CNH consult in accordance with consult business rules established by the facility, VISN, and VHA. Ensures that Veterans placed in the CNH program meet VA eligibility requirements. Maintains a current and historical roster of program participants to enable tracking of the authorization for and utilization of services. Completes monthly oversight visits to Veterans in CNH facilities. Provides direct clinical services to Veterans. Consults with CNH facility staff to address and resolve Veterans' concerns as they develop. Ensures services are provided with attention to the dignity of the resident and consideration of the residents' capacity to understand treatment options. Provides review, follow-up, and appropriate documentation of all complaints regarding the program. Coordinates a quarterly satisfaction survey of CNH residents. Serves as the Contracting Officer Representative (COR) for all assigned CNH contracts and fulfills all COR obligations. Reviews and approves claims for CNH services within seven days of receipt. Collaborates with Community Care and Payment Operations Management staff to ensure that payments to CNH facilities are made in a timely manner and in compliance with VA standards. Conducts psychosocial assessments of a wide variety of individuals from various socioeconomic, cultural, ethnic, educational, and other diverse backgrounds. Identifies high-risk patients and provides case management services. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences, and serves as a liaison to family members. Coordinate community-based services, including information and referral for additional services from other VA programs, other government programs, and community agency programs. Provides consultation services to other staff regarding the psychosocial needs of Veterans and the impact of psychosocial problems on health care and adherence to treatment plan.
