The registered nurse provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being. Provide assessment and intervention services for patients enrolled in the Substance Use Disorder Recovery Program and Opiate Substitute Clinic. Patient populations include Veterans who present with the full range of DSM-V mental disorders, including mood disorders, anxiety disorders, psychotic disorders, and substance use disorders Based on education and experience, the nurse may be assigned responsibilities as a preceptor to nursing students and/or new employees Exhibits positive approaches to management of complex client care in the Mental Health Outpatient setting RN functions as part of an expanded behavioral health interdisciplinary team working together to develop innovative, evidenced based Veteran centered treatment plans Demonstrates ability to assess and document the needs of outpatient mental health patients in a self-directed manner for both walk in patients and phone triage. Reassesses patient changes and accordingly documents patient care Acts as a consultant in clinical practice or in area(s) related to practice role responsibility Provides education for MHS (Mental Health Service) patients, as well as, assesses outcome. Develops creative and innovative programs for patient/family education. Establishes patient /family teaching for MHS patients with interdisciplinary involvement She/he assists the provider with addressing care needs including medication administration/management, health education, telephonic follow up and/or making notifications, referrals and care transitions Assists with intakes and coordinates with other disciplines to correct scheduling issues to provide more efficient access to mental health services Initiates strategies for policy development.
