This Health Technician vacancy exists in the Patient Aligned Care Teams (PACT) outpatient clinics. The Health Technician is responsible for the clinical, laboratory and/or clerical functions of patient care in order to promote timely and efficient patient-centered care. The Health Technician is a member of the Core Team (Patient, Primary Care Provider, RN Care Manager and LPN). The Health Technician duties are as follows: Ability to maintain and perform Veteran charting using standard approved medical terminology for human body, physical and emotional reactions, nursing care, etc. Schedules appointments using VISTA clinic appointment management. Ability to manage medication reconciliation list and distribute to Veterans. Enters information regarding care rendered and condition and progress of Veterans. Identifies and documents problems or potential problems and facilitates process analysis to alleviate problems. Participates in team weekly meetings and Individual Development Treatment (IDT) meetings in person or through V-tel. Meets the needs of customers while supporting the Medical Center and Service missions. Consistently communicates and treats customers (Veterans, visitors, volunteers, and all Medical center staff in a courteous, tactful, and respectful, manner). Provides a customer with the consistent information according to establishes policies and procedures. Handles conflict and problems in dealing with the customer constructively and appropriately. Uses MS Word or other comparable word processing software to execute several office automation functions such as storing and retrieving electronic documents and files, activating printers, inserting and deleting text; formatting letters, reports, and transmitting e-mail. Uses the Veterans Health Information & Technology Architecture (VISTA) to access information in the Medical Center Computer System Follows applicable regulations and instructions regarding access to computerized file release of access codes as set in the computer access agreement that employees signs. Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, federal regulations, and VA statutes and policy. Performs Veteran care procedures such as collection of specimens, applying dressings, adjusting simple supportive equipment. Performs simple procedures such as collection of blood samples or other fluid samples from ambulatory veterans, application or sterile dressings to uncomplicated wound or similar Veteran care. Uses proper sterile techniques for collection of samples. Verifies correct labeling and Veteran identification. Records test time and intervals as appropriate. The Health Technician collaboratively with the Core and Expanded Team (family / caregiver, internal and community-based services involved in providing care to the patient) in setting priorities and deadlines for Veteran care. HT takes direction from all members of the core and expanded teams in order to provide timely and safe patient care. The Health Technician is responsible for the laboratory services provided in the CBOCs. The applicant will be required to function under the supervision of the RN Team Leader. This position requires scheduling flexibility that may include some evening and Saturday tours. Works sites will include Northport VAMC, Valley Stream CBOC, East Meadow CBOC, Patchogue CBOC, and/or Riverhead CBOC. Preferred Criteria: Med/Surgery, Geriatric or Extended Care experience background required, with at least 2 years experience preferred Phlebotomy experience preferred Current American Heart Association BLS certification Ability to prioritize and multitask in a fast paced environment Applicant must be self-motivated, possessing critical thinking and interpersonal relationship skills Work Schedule: Monday thorough Friday, 7:30 AM- 4:00 PM - Include weekends and/or evening tours Compressed/Flexible: N/A Telework: Not Available Virtual: This is not a virtual position.
