The Patient Aligned Care Team (PACT) II Medical Sub-specialty Clinic Staff Nurse demonstrates leadership, experience, and ability to management of complex patient care. Follows the ANA Code of Ethics for Nurses, incorporating and implementing the requirements of the other qualification standards including: Leading and organizing delivery of care to assure continuity of care and peer accountability for practice, including access to care and discharge planning Using advanced clinical knowledge/judgement to promote staff involvement in planning, decision making, and evaluating outcomes. Functioning as an expert in clinical practice and/or in areas related to the assigned roles and responsibilities Systematically evaluating current practice, and formulating outcomes for groups of patients/residents and/or organizational processes within area of expertise. Guiding, developing, and supporting staff from a leadership perspective. using professional standards of care, scientific evidence, and practice to evaluate programs and/or service activities Provide direct patient care in outpatient or inpatient setting based on need to include but not limited to infusions, medication, care interventions, and communicate with interdisciplinary treatment team. The PACT II Medical Sub-specialty Clinic Staff Nurse works collaboratively with the Medical Sub-Specialty Providers implementing the plan of care for Veterans with acute problems. Care is multidisciplinary, including collaboration with Dietetic Services, Business Operations, Social Workers, PACT staff, other support and ancillary medical staff, and various providers in the community, and incorporating best practice from research studies.
