We are seeking Board Certified/Board Eligible Internal, Family or Geriatric Medicine physicians with acute or post-acute experience to join ourexpanding team. This position involves the physician consultation services for the acute inpatient service to support the hospitalist service. In addition, the geriatrician will maintain small SNF practice in coordination with dedicated geriatric advanced practice clinicians (APN/NP/PA) team. Palliative training is plus.
Essential Job functions include:
Examinations of patients admitted to acute care hospital and skilled nursing facilities
Consultation Services will include the care and treatment of the patient, geriatric syndrome and the disease that affect them. Additionally, advanced care planning guidance, transitions of care assessment, geriatric assessment for identifying care needs, and risk assessment.
Provide guidance and education regarding geriatric services for Advance Practice Clinicians and Hospitalist
Coordinating all tests, examinations, and treatments with multidisciplinary
Interacting with primary care physician for transitions of care.
Maintaining hospital records and medications.
Arranging for transitions in care and the follow up care for patients once they leave the hospital
Communicate effectively with physicians, patients and family members.
