| The LPN is responsible for patient care, patient assessment, documentation, medication administration and performing all elements of the nursing process. The LPN serves as a responsible member of the nursing team and appropriately interacts with Veterans, family members, clinicians, and supportive personnel related to patient care activities and outcomes. The work involves a variety of patients of diverse cultural, spiritual, and education backgrounds. Duties include but not limited to the following: 1. Applies the nursing process to administer holistic patient care. Competently performs all duties within the scope of licensure however completed work may require routine or detailed higher level of review. 2. Administers medications via all routes, except intravenous, and carries out basic treatments as ordered, monitors patients' response to medication and treatments, reports changes and documents effectiveness. 3. Functions as a member of the team by participating in planning, implementation and evaluation of nursing care using a focused assessment to care for the adult patient with a variety of physical and/or behavioral problems. 4. Participates in providing or reinforcing effective patient education to both patient and their family in relation to disease process, medications and treatments, procedures and surgeries, equipment and devices or any education deemed necessary. Documents the effectiveness of the education. 5. Assists the patient in activities of daily living toward the attainment of rehabilitative/restorative goals. 6. Performs escort and sitter duties based upon the care needs of the veteran. 7. Assess life threatening conditions and initiates and/or assists with CPR or other appropriate interventions including prompt notification of an RN or MD/DO to provide appropriate direction and intervention. 8. Demonstrates initiative during Condition A & C or Code Blue emergencies by anticipation and assuming essential tasks and applies the knowledge to subsequent clinical emergencies. 9. Assists others in the use of preventive techniques or measures when interacting with patients displaying disruptive, acting out behaviors and practices the use of the least restrictive environment. 10. Adheres to Medical Center policies and procedures when caring for patients on suicidal codes or in restraints or in seclusion. 11. Applies ethical principles to practice and seeks assistance to address ethical issues with care. 12. The ability to work independently in the accomplishment of a wide variety of duties including setting priorities and coordinating work. 13. Ability to communicate effectively and professionally with both internal and external customers. 14. Ability to identify customer concerns, perform the tasks required to resolve the issue accurately and timely and follow-up as necessary to ensure a satisfactory resolution. 15. Ability to organize work, set priorities, and delegate tasks/responsibilities in order to provide and expedite patient care. 16. Must be customer oriented and conduct themselves in a professional, courteous and friendly manner at all times. 17. Collaborates, communicates, sets priorities and organizes assigned work in order to meet deadlines, ensuring compliance with established processes, policies, and regulations. 18. Communicates tactfully and effectively, both orally and in writing on order to meet program objectives. 19. Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards or supply deficiencies and reports to appropriate personnel for corrective action.
