The Social Work Program Coordinator will be assigned to the Suicide Prevention Program for the Columbia VA Health Care System Mental Health Service Line, associated Community Based Outpatient Clinics, or using Tele-Work or Tele-Mental Health processes. As a Suicide Prevention Coordinator, the Social Work Program Coordinator will serve as the primary point of contact and coordinator for the National Category I High Risk for Suicide Flagging program within the Suicide Prevention Team. The SPC will provide high quality mental health care and services to assigned Veteran patients. This includes active case management and care coordination, clinical assessment and treatment, and coordination of linkages to other appropriate VA or community service providers/agencies as needed by the client. This is accomplished in cooperation with other members of the treatment team and/or other interdisciplinary teams as appropriate. The incumbent is expected to function above the Full Performance Level and demonstrate the capacity to work independently in the clinical setting. The incumbent is delegated the responsibility and authority for ensuring local planning and coordination of mental health care and support to Veterans who are at high risk for suicide. The incumbent assists the medical center administration and staff with improving identification of Veterans at risk for suicide and promotes early access to mental health care. The SPC is a subject matter expert and point of contact for any suicide related matters, in particular, care coordination for Veterans identified as part of the National Category I High Risk for Suicide Flagging program. The incumbent is responsible for program development, implementation, follow-through and ongoing review of the National Category I High Risk for Suicide Flagging Program for CVAHCS and the surrounding CBOC's, as well as the Suicide Prevention Program as needed. This position is eligible for part-time telework. Work Schedule: Monday - Friday, 8:00 am to 4:30 pm. Subject to change based on the needs of the agency. Financial Disclosure Report: Not required
