Senior Social Worker serves in a senior or lead capacity with experience that demonstrates possession of advanced practice skills and judgment. The Senior Social Worker performs assigned functions independently at a VA facility and in the community. HUD-VASH is an intensive case management program for chronically homeless Veterans aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system. The primary responsibilities of this position include clinical and administrative program oversight and monitoring; serves as one of the primary contacts with the local Public Housing Authority's (PHA's); provides clinical and psychosocial case management services; and ensures that the Veteran's care is coordinated with the VA Medical Center, community health or mental health providers, and agencies that serve homeless populations. The Senior Social Worker in this position will utilize various treatment modalities of individual and group work, casework, community organization, and community outreach; participate in program planning and development, and process improvement activities; and participate in annual community based events. Senior Social Worker Utilizes a variety of advanced clinical skills adapted to both inpatient and outpatient clinic settings and community visits and outreach; incorporates the VA ICARE values within the position functions assigned and use advanced practice clinical skills for a specialized Veteran population where care is provided. VA program assigned defines the level of interventions, functions, and responsibilities. Senior Social Worker performs the functions throughout medical center and community. Provide high-risk triage case management psychosocial assessment, case management, psychosocial assessment, psychosocial counseling, psychosocial education, resources information and referrals. Communicates with team members the functions they are responsible. Review and complete continued education annually specific to program population to maintain or enhance clinical competencies in addition to maintaining continued education requirements. **Certain locations are authorized to offer recruitment/relocation incentives for eligible and highly qualified applicants. Work Schedule: Full Time, 40 hours a week. Varied schedules to be discussed during interview.
