The Eastern Colorado Veterans Health Care System, Mental Health Service Line is currently recruiting for one (1) Social Work Associate for the providing coverage for the HUD-VASH program within the Homeless & Vocational Services, Aurora, Colorado. You will be required to operate a government as part of your duties. You must provide a copy of your unrestricted state issued driver's license with this package. This position is assigned to the Homeless and Vocational Section of the Mental Health Service at the Eastern Colorado Healthcare System Denver location. Work with veteran's assigned Social Worker to determine needs of veterans and family members. Will provide consultation/education to veterans and families on community housing resources and VA and non-VA benefits. The position includes acting as a consultant to other team/staff members on housing & benefits. Acts as liaison between VA and community resources, to include marketing and public relation duties as needed. Provides case management to veterans and families, as needed, throughout the continuum of care. Provide education and facilitate information groups as appropriate. Maintain data and statistical compilations to comply with JC, CARF, VA and medical center policy and procedures and will document contacts and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when referring a veteran for care. Maintain familiarity and responsibility for all performance measures relating to the HUD-VASH program.
