The Home Based Primary Care Nurse is directly responsible to the Program Director for Home Based Primary Care. This Nurse Practitioner is responsible and accountable for the initial and continuous assessment for a culturally diverse veteran patient population in the home setting. In addition to assessment, this nurse evaluates, plans, implements, coordinates care and confers with Home Based Primary Care team in delivering care to veterans in the home care setting. This individual also provides teaching to the patient and caregiver in ways of managing and delivering nursing care in the home; monitors patient's condition and the care required in the home and collects blood and other specimens required for monitoring the effectiveness of interventions and/or disease progression. This individual provides needed assistance and consultation for home care referrals. The Home Based Primary Care Nurse possesses the knowledge and skills to effectively and collaboratively apply all aspects of the nursing process within the area of assignment. Work Schedule: Monday - Friday, 8:00am to 4:30pm Financial Disclosure Report: Not required This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
