This vacancy will remain open until filled. The first cut-off date is 12/21/2020. Additional applications will be referred as needed. Senior Social Worker (Hospital Liaison) serves as the lead clinician and coordinator for providing services to Veterans enrolled at El Paso VA Health Care System who require inpatient psychiatric services. Responsible for authorizations for hospitalizations within the community; support/coordinate warm hand-offs of Veterans in crisis. Duties include, but are not limited to: Provides clinical consultation and guidance to others on the Hospital Liaison team, colleagues outside the team and in the community as necessary. Recommends evidence-based services for high-risk Veterans and be able to provide brief counseling to veteran in crisis as the appropriate services are connected. Act as a role model of effective social work practice, and be able to provide clinical supervision for social work licensure or certification and supervise social workers who are not independently licensed. Responsible for authorizations for hospitalizations within the community. Support and coordinate warm hand-offs of Veterans in crisis with the Suicide Prevention Program. Direct oversight of the mental health administrative responsibility for starting follow-up for all psychiatric discharges (e.g., first day-of-contact with the Veteran post-discharge, timely transfer to SPC team for enhanced care follow-up assignment), safety plans for hospitalized suicidal patients, making provider assignments for Veterans released from the hospital who are not currently assigned, and ensuring that coordination of care upon discharge is consistent with the Uniform Service Handbook timelines. Involved, alongside the supervisor, in tracking performance and improving service of the hospital liaison team. May be called upon to assist with an emergent crisis hospitalization when on site and no other provider can be readily available to assist. Will travel and meet with Veterans at different psychiatric facilities to introduce him/herself and educate the Veterans about his/her role, give them proper discharge instructions, participate in treatment team meets are requested or direct another Hospital Liaison team member to participate if unavailable, and advise outside hospital staff of VA policy, contacts and expectations. Work Schedule: Monday thru Friday 8:00 a.m. to 4:45 p.m. Financial Disclosure Report: Not required This is a Bargaining Unit position.
