The Kansas City VA Medical Center is seeking 1 Social Worker to work in the Emergency Department. Responsible for coordination and consultation as needed with mental health clinicians regarding the development of treatment plans that include family involvement. Set achievable treatment goals with the Veteran/family and other providers. Facility subject matter expert on VA and/or community resources regarding family services. Responsible for educating providers on the available family mental health services and for assisting them in establishing appropriate referrals based on the Veteran's needs (or that of the Veteran's surrogate decision-maker). Collaborate and consult with other mental health service providers when needed. Experienced in making rapid assessments and developing crisis management plans for admission to mental health inpatient programs. Demonstrates the ability to teach and mentor staff and students in the special area of practice Demonstrates an understanding of the range of treatment and skills needed for all Veterans with mental disorders, including those with severe and persistent mental illness and co-occurring disorders. Knowledge of VA benefits including information about referral to and provision of family mental health services within Kansas City VA Medical Center, other medical centers in VISN 15, and community care options. Responsible for utilizing, developing, and implementing methods for measuring effectiveness of practice and services in his/her specialty area. Help with facilitating patient flow in the emergency department when needed. Identification of surrogate decision maker when appropriate. Scheduling transportation for follow up appointments and other needed VA services when appropriate. Assisting with scheduling home health, skilled nursing placement for emergency department patients when consulted. Assist with reporting elder abuse and child abuse. Serve as a resource and coordination of care for Intimate Partner Violence Acute crisis intervention/counseling in the Emergency Department. Work Schedule: Schedule will be six 12 hour shifts between 1000 and 2200 and one 8 hour shift between 1400-2200 per pay period. There are 2 (two) weekend shifts each pay period. Weekends and holidays will be shared equitably between the two social worker positions for the ED being filled. On-call required. Overtime compensation paid per the facility collective bargaining agreement. Financial Disclosure Report: Not required
