In this position you will have the opportunity to work as an Ambulatory Care Clinical Pharmacists embedded within primary care and/or specialty clinics providing disease state management services while working under collaborative drug therapy management (CDTM) agreements. Current CDTM agreements include diabetes, anticoagulation, lipid management, hypertension, tobacco cessation and others where clinically needed. Ambulatory Care Clinical Pharmacists actively manage medication therapy, complete medication reviews, collaborate with providers to optimize medication therapy, improve medication adherence, promote patient activation and wellness, prevent disease, and improve outcomes. Our embedded pharmacists are an integral member to the care team and provide additional patient-care services such as transitions of care outreach, polypharmacy consults, population health gap assessments (i.e. statin-use and adherence interventions) and may provide ancillary services depending on patient-care volume. This position is at Banner Health Center Peoria (13640 N Plaza Del Rio Blvd Peoria, AZ 85381). Banner is expanding its focus from being a hospital company to a clinical quality company focused on population health management. Clinical Pharmacy Services are also included in this robust new service line and are responsible for the management of the Banner formulary, development of standardized system wide clinical pharmacy services, and management of adverse drug event preventions. Clinical Pharmacy Services provides collaborative drug therapy management and educational interventions in patient care to optimize medication therapy, improve outcomes, promote wellness, and disease prevention. POSITION SUMMARY 5. Participates in ordering or prescribing, administering and monitoring of all medications utilized. Internal customers may include Patients, Nurses, Licensed Independent Practitioners, Administrators and other health care team members for the purpose of integrating services, improving patient care and ensuring effective communication systems. External customers may include local, state, and national pharmacy organizations, State Board of Pharmacy, colleagues and peers, educational institutions and pharmaceutical representatives. A degree in Pharmacy from an accredited College of Pharmacy. Completion of an accredited PGY1 pharmacy residency program or three years of previous clinical pharmacy experience in the ambulatory care setting. Possesses good written and verbal communication skills for interfacing with all levels of staff, physicians, patients, and other contacts.
