The Hospice/Bereavement Social Worker is responsible for providing for the psychosocial needs of veteran residents of the Hospice Unit and serves as a member of an interdisciplinary care team. As the facility bereavement provider, duties will involve support for all program services, following the established program national guidelines, while working in partnership with other professionals providing bereavement services (Chaplain, Decedent Affairs).
