CN II – Ambulatory Clinic Nurse
San Leandro, California – Fairmont Hospital
Per Diem, Services As Needed
In this role, you will perform professional nursing duties for the outpatient clinical areas. You will treat and educate patients, their families and the public regarding various medical conditions; record medical histories and symptoms; perform diagnostic tests and analyze results; and operate medical equipment. You will also be responsible for administering treatment and medications; making assessments and creating a plan of care; performing care coordination and case management; and participating actively in the team-based care model. In addition, you will provide lead direction to other nursing and paraprofessional personnel and perform other duties as assigned and required by the assigned clinical setting.
A graduate from an accredited Registered Nursing Program
A Bachelor’s degree or higher in nursing preferred
Six months of full-time recent experience as an RN in a comparable area of specialty
At least two years of current experience as an LVN practicing in the ambulatory clinics at Alameda Health System may substitute for six months of full-time recent experience as an RN
A valid license to practice as a Registered Nurse in the State of California
BLS – Basic Life Support Certification issued by the American Heart Association
The ability to provide existing NPI number or obtain a new one if needed
