Foundation Medical Partners is looking for a child psychiatric APRN to join our growing practice. 

Responsibilities of the Pediatric Psychiatric APRN include:

• .8 - 1.0 FTE,  4 or 5 days a week. 
• Participate in a collaborative care model of treatment:
  • receive referrals from our primary care physicians seeking assistance with the treatment of behavioral health disorders. 
  • evaluate, prescribe medication as appropriate, stabilize the patient and then return the patient to their PCP for ongoing treatment and will refer to community resources if the patient’s acuity requires comprehensive, long term treatment. 
  • Work collaboratively with the PCP practices and offer consultation as needed. 
  • Work collaboratively with a multi-disciplinary team of professionals including physicians, physician assistants, nurse practitioners, therapists and nurses. 
• Candidate must be familiar with integrated care treatment models and be willing to implement services accordingly.
