The Clinical Nurse Navigator is a registered professional nurse with oncology-specific clinical knowledge who offers individualized assistance to patients, families, and caregivers to help overcome healthcare system barriers. Using the nursing process provides education and resources to facilitate informed decision-making and timely access to quality health and psychosocial care throughout all phases of the cancer continuum. The navigator's main focus and responsibilities will include: Serve as patient and family advocate. Assess patient needs throughout navigation, matching unmet needs with appropriate services and referrals. Identify potential and realized barriers to care and facilitates referrals as appropriate to mitigate barriers. Facilitates timely scheduling of appointments, diagnostic testing, and procedures to expedite the plan of care and to promote continuity of care. Participates in coordination of the plan of care with the multidisciplinary team, promoting timely follow-up on treatment and supportive care recommendations. Provides education and reinforces to patients, families and caregivers the significance of adherence to treatment schedules, protocols and follow-up. BSN required. Two to three years of oncology nursing experience required. One year of customer service experience in a healthcare setting preferred. Licensed by the NJ State Board of Nursing as a Registered Nurse and BLS provider required. Oncology or navigation certification required within two years of employment.
