RN - Chronic Care Management YVFWC Central Administration Toppenish, WA Full-time, 40 hrs/wk
This position is located in sunny central Washington State. We are interested in all qualified candidates, including those outside the area who are open to relocating to the Yakima Valley. Relocation allowance is available.

This is a unique role within our Managed Care team that is responsible for patient populations with chronic illness and/or multiple co-morbidities. Work is accomplished in coordination with multidisciplinary teams and serves as the central link to an identified population of patients. Through skilled non-face-to-face assessment, education, collaboration and coordination of healthcare and community resources, you'll help patients gain self-efficacy/management skills, achieve optimum functional health status, and quality of life. You'll assist patients, families, staff, and systems to achieve high quality, evidence-based, cost-effective, and patient-focused outcomes.

A compassionate RN with knowledge of chronic care management, risk stratification and population health, utilization management, quality improvement, care coordination, and cost management.
Ability to work independently with excellent time management and prioritization skills.

A clear communicator with excellent written and verbal communication skills.
Knowledge of human service programs and the ability to develop effective working relationships with clients, social service providers, and partners.
Flexibility and willingness to work some evenings and weekends; differential pay for qualifying shifts.
What You'll Do
Review patient charts and identify those with two or more chronic conditions, and contact them by phone to provide coaching and education, help them set goals, and identify resources in the community to assist them in better managing their complex health issues and reducing healthcare costs.
Identify problems that could lead to an emergency/crisis situation and takes appropriate action to de-escalate.
Use knowledge of illness, disease processes, diagnosis and treatments to promote continuity of care and oversee the coordination of care including transitions to achieve cost-effective, quality care.
Conduct effective, ongoing evaluation of care plans, making adjustments as appropriate.
Make periodic outreach calls to each patient to address any needs related to chronic conditions, confirm medication adherence, and assure alignment across the patient's entire care continuum.
Maintain client files, following program policy and procedure, with attention to detail and accuracy.

Bachelor's Degree in Nursing, OR Associate's Degree + 2 years' directly related RN experience can be substituted for the bachelor degree.
WA State RN license or ability to obtain before starting work; obtain Oregon RN license within six months of hire.
At least 3 years of directly relevant chronic care management experience, including working with Medicaid, Medicare and private insurance beneficiaries with two or more chronic conditions.
Driver's license and proof of auto liability insurance coverage.
Chronic Care Professional (CCP) certification or the Iowa Chronic Care Consortium Certification is a plus but not required.
Bilingual English/Spanish skills are useful but not required. Differential pay for Spanish fluency with a score of 10 or higher on the ALTA language scale.
