Summary of Job Function: Under the direction of the VP, Southern Region Business Development & Director, Project Management at Business Development Division, the Program Coordinator will operate, manage and support regional motor vehicle accident (MVA) initiative and other initiatives as determined by southern region leadership. For MVA initiative, the goals are to provide post hospital utilization care coordination for patients involved in motor vehicle accident for continuity of care and revenue retention within the system. The key responsibilities include but not limited to (1) Communicate and collaborate with hospital departments and key physician practices to navigate and refer patients for appropriate outpatient testing or specialists for consultation based on discharge referral recommendation. (2) Assist in scheduling testing and specialist referral appointments recommended. (3) Works collaboratively with all members of the healthcare team to improve patient care and satisfaction. (4) Serve as a point of contact for patient in all related MVA related tasks. For other regional key initiatives, the coordinator will provide all levels' care coordination as needed. Program coordinator will work with medical group, community partners, hospitals and southern region leadership to track and monitor outcomes for outpatient modality and specialist referrals to hospital, joint ventures or ambulatory centers. He/she will perform under the concepts of the High Reliability Organization guidelines and is also responsible for reporting, identify safety issues such as hazardous environments (i.e. damaged floors/walls/ceiling tiles/unsecured areas) and medical errors.
Job Qualifications: Bachelor's degree required. Strong knowledge of medical terminology and experience as a coordinator in health related field is a must. Medical group and hospital working experience and familiar with RWJBH outpatient imaging scheduling process is preferred. Must possess excellent organizational, time management, strong customer service skills and interpersonal skills. Strong sales skills, excellent communication and formal presentation skills required. Well-developed administrative abilities and leadership skills necessary. Proficient in Windows-based computer programs such as Word, Excel, PowerPoint, Outlook, etc.
