Primarily responsible for performing functions of Clinical nutrition service, such as nutrition assessment, modified diet formulations and self-management training.
Assess high risk patients identified by nursing from patients health history form, physicians, and other allied health care professionals. Assessment may include:
Calculation for energy and proteins needs based on body mass index (BMI), height/weight, and ideal body weight (BMI).
Determination of visceral protein and somatic protein depletion/malnutrition.
Analyzing electrolytes, micronutrients (vitamins and minerals), macronutients (carbohydrate/protein/fat), and other laboratory data applicable to normal and diseased pathophysiology.
Calculate estimate fluid requierement based on age, hydration, and clinical status of patients.
Determinations of obesity based on BMI with appropriate level of grading.
Follow Nutrition Care Plan to reassess and change plans as necessary.
Calculate and document intake analysis.
Educate patients and family members on modified diet.
