Serves as the Physician Leader acting to engage providers in the development of a Value-Driven Healthcare Organization. Provides physician leadership guidance that assists IPM healthcare providers to achieve consistently high quality patient care, with greater efficiency, at lower cost. Drives implementation of Evidence Based physician practices (including utilization of technology) to reduce variation in care and improve patient outcomes across Acute and Ambulatory Care settings. Provides leadership to foster physician alignment and performance to exceed patient, payer, network and regulatory performance expectations, resulting in better care, lower costs and increased revenue
Provides leadership in the development of Evidence Based Care Protocols for ambulatory providers to achieve better patient outcomes and reduced costs. Drives provider consensus for adoption of evidence-based medicine. Helps guide the development and reporting of quality metrics. Evaluates provider performance through data analytics, structured reporting and provider performance scorecards.
Supports development of tools that utilize Evidence Based Care Protocols and leverages technology to align physician practice behaviors with organizational initiatives in higher quality, lower cost care. Improve performance on Quality Incentive metrics to optimize incentive revenue. Identifies barriers and creates processes/action plans to eliminate those barriers. Works with ACO Teams and Strategic Payer Council as the Physician Leader in the development of IPM Provider Networks and Quality performance metrics to ensure physician alignment with organizational initiatives and population health goals. Act as the IPM Clinical Leadership to collaborate in the develop Evidence Based Care Protocols and drive ACO Friendly Hospital Initiatives to ensure IPM is partner of choice in our markets for Value Based Care programs.
Collaborate with in patient teams (Leadership, Physicians and Care Management) to optimize and coordinate care across the continuum resulting in high quality, cost-efficient outcomes. Provides support to improve the quality of clinical documentation in the ambulatory care setting to ensure accurate representation of the acuity of the patient condition and the care provided.
Works directly with the Acute Division Chief Medical Officer, ACO Executive & VP IPM to coordinate Triple Aim initiatives across IPM Acute Division.
Assists with the development of Provider recruitment plans for each Market to attract and retain high quality providers to meet the needs of the communities served.
Develops Physician Leadership in each Market to lead the local quality improvement initiatives, clinical efficiencies and growth strategies
