As part of a dynamic interdisciplinary team, oversees inpatient care to ensure hemodynamic optimization and adherence to quality standards and core measures. Manages new referrals for Advanced Heart Failure (AHF) therapies in the inpatient and outpatient settings and completes History and Physicals, orders diagnostic testing, and prepares patient summaries for presentation to AHF Multidisciplinary Committee. Provides primary outpatient medical management of patients with AHF therapies, addressing routine follow-up and acute concerns. Reviews outcome data and critically analyzes quality metrics. Develops quality improvement plans and coordinates implementation with the interdisciplinary team. Provides advanced education to staff members, patients and families about cardiac transplant and Ventricular Assist Devices.
Master's Degree required. Pennsylvania Board Certification as a CRNP or Physician Assistant required. 3 years of experience with cardiac transplant and/or mechanical circulatory support patients in the inpatient and outpatient care environments required. Certification as a Clinical Transplant Coordinator required or must obtain within 18 months from date of hire. ACLS and BLS certification required.
