The Lexington Geriatrics & Extended Care Service is recruiting for a full-time physician for the Geri-PACT program. This program includes Geriatrics primary care clinic as well as three specialty clinics addressing Falls, Palliative and Hospice care as well as Dementia Evaluation. Office location will be at the Leestown Division.
The Geri-PACT provider is responsible for the longitudinal management of the health care of elderly patients with a range of needs. At one end of this spectrum are those relatively healthy seniors who desire the services of a provider whose professional interests are focused exclusively on care of the elderly. At the other end of the spectrum are the most frail, complex, and challenging elderly patients who require a primary care provider (PCP) with specialized-clinical expertise plus familiarity with the range of community services and other support mechanisms necessary to provide integrated care to those of advanced age whose lives are marked by coexisting functional, social, cognitive, and medical problems. Other duties may include providing high quality patient centered care to veterans across other GEC programs with emphasis on the Hospice Palliative Care Inpatient and Outpatient Consultative Service as well as the Community Living Center (a skilled, rehab, hospice inpatient facility) when the need arises during staffing deficiencies.
The physician will have administrative, clinical, and educational responsibilities. The physician is expected to demonstrate knowledge of established and evolving biomedical, clinical and social sciences and the application of their knowledge to patient care and the education of others. The physician will ensure practice-based learning and improvement, and he/she will be expected to be able to use scientific evidence and methods to investigate, evaluate, and improve patient care. The physician is expected to demonstrate interpersonal and communication skills, and behaviors that reflect a commitment to continuous professional development, ethical practice, and understanding and sensitivity to diversity. The physician is expected to demonstrate both an understanding of the context and systems in which health care is provided, and the ability to apply this knowledge to improve optimizing health care.
Board eligible physician in Internal Medicine or Family Practice with CAQ in Geriatrics as well as ABIM Certification in Hospice & Palliative or active and ongoing experience in Hospice and Palliative Care is highly preferred.

Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine.
Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
